What It Is

Graded Motor Imagery (GMI) is a staged rehabilitation approach that retrains the brain's neural networks involved in movement and pain. It is based on the understanding that chronic pain can change how the brain 'maps' the body — and that retraining this map can reduce pain. GMI is particularly used for conditions like CRPS and AMPS, where the brain's representation of the affected limb may be altered. It involves progressively exercising the brain's motor system through mental imagery, not physical movement.

How It Works

GMI progresses through three stages: 1. Left/Right discrimination: Identifying whether a pictured hand or foot is left or right. This activates the brain's motor cortex without moving the limb. 2. Motor imagery: Imagining moving the affected limb (without actually moving it). This further activates motor networks. 3. Mirror therapy: Using a mirror to create the visual illusion that the affected limb is moving normally, which helps 'retrain' the brain's body map. Each stage is progressed only when the patient can do it without a significant increase in pain.
