A post-viral condition causing persistent symptoms — fatigue, brain fog, pain, and autonomic dysfunction — following COVID-19 infection.
Overview
Long COVID (also called Post-COVID Condition or PCC) refers to symptoms that persist for weeks or months after a COVID-19 infection. It can affect anyone — including those who had mild initial infections — and symptoms can be wide-ranging and debilitating. Long COVID shares many features with other post-viral illnesses like ME/CFS, including profound fatigue, post-exertional malaise, and autonomic dysfunction. Many Long COVID patients also develop POTS, small fiber neuropathy, or other chronic pain conditions. While research is still emerging, Long COVID has brought much-needed attention to post-viral illness and the millions of people who have been living with similar conditions for decades.
Common Symptoms
Common symptoms include: • Profound fatigue that worsens after exertion (PEM) • Brain fog and cognitive difficulties • Shortness of breath and chest pain • Heart palpitations and rapid heart rate (POTS-like symptoms) • Chronic pain and muscle weakness • Loss of taste or smell • Sleep disturbances • Digestive issues • Mood changes (anxiety, depression) Symptoms can fluctuate and may affect multiple body systems simultaneously.
Diagnosis
Diagnosis is based on: • History of COVID-19 infection (confirmed or suspected) • Persistent symptoms lasting 3+ months • Ruling out other conditions • Assessment of individual symptoms (cardiac, neurological, autonomic) There is no single test for Long COVID — it is a clinical diagnosis.
Current Treatments
Treatment focuses on symptom management: • Pacing and energy management to avoid post-exertional malaise • Treatment of specific symptoms (POTS, neuropathy, breathing difficulties) • Graduated rehabilitation (only when appropriate and carefully monitored) • Mental health support • Treatment of co-occurring conditions • Multi-disciplinary care from Long COVID clinics (where available) There is no standard treatment protocol yet — care is highly individualized.
Research Summary
Long COVID research is one of the most rapidly growing fields in medicine. Studies are exploring viral persistence, immune dysfunction, microvascular damage, and autonomic nervous system involvement. This research is also advancing understanding of other post-viral illnesses like ME/CFS, which have been under-studied for decades.
